Can a high sperm count and low sperm motility cause problems for a couple during pregnancy?

Hi doctor,

I have a problem with fertility, I married about two years back and still, I do not have a child. For the last three months I have been taking the medicine Coenzyme q 10 after consulting the doctor and yesterday I did semen analysis again. The result shows my sperm count increased highly (194) and motility is still very low. I will attach those details here. My query is whether it is fine to get pregnant if the sperm count is high, even if the motility is very low, or if there is no chance. Please help. Thank you.

I read your query and understand your concern.

Let me start by assuring you, if you have no other significant problems then God has decided the right time and when it comes to you, you will surely have a baby.

I would like to tell you the possible causes of semen problems. If you notice in the report, the count is fifty-nine million/ml. This is normal. But the viscosity of semen is less than sixty minutes, it should be more than thirty minutes. There is no infection at present as WBC is normal, but the sperms that are effective are only nine percent.

This means there is a good number of sperms being produced in the testis ( volume of the testis is normaland healthy ) but on the way out to the exterior, there are lots of glands producing secretions for the sperms' health and movements. One or more of these glands are infected, so the secretion is not healthy, so the thickness of discharge is more ( just like, after a serious cold initially running nose, later as the cold is gone, the discharge is thick and yellowish), so the sperms cannot move in it.The normal count of active motile sperms is thirty-two percent, which is nine percent here.

So the egg which waits in the tube will wait, but the sperm in the uterus or cervix and vagina will not have the energy to swim up and it dies before reaching there.

Now this is what we can do. Do not plan to correct the symptom (low motility), think of clearing the underlying problem (infection).

Sperms are being produced every day, the sperm tested today was produced in the body three months ago, this is the life cycle of sperm. So do not repeat the semen for two months or so. We can surely correct the motility problem. Do not get carried away by the medicines in the market which claim to make the sperms into rockets and bullets or whatnot. There are no such medicines, they are all health supplements (you can check on the internet), and even the medicine you are taking is the same.

To confirm if the infection is there or not, tell him to press lightly on the testis and just above it, if it is painful, or if he has complaints of pain on passing urine after sex or is having premature ejaculation, cannot control semen for long, have a course of antibiotics too. The best is Capsule Doxycycline 100 milligrams twice a day for two weeks, followed by

2. Tablet Levofloxacin 500 milligrams once a day for two weeks.

I am sure he will not have those symptoms and a semen report after the medication will be excellent.

3. Have a cold water bath many times, or at least dip the testis in cold water for fifteen mins two-three times a day

4. Drink a lot of cold things like - water, juice, and milk.

5. Avoid smoking (or even standing next to someone smoking), and avoid alcohol.

6. Practice some mind relaxation. I know no one has time, just 15 minutes in morning meditation or yoga or just keep the phone off and look at the sky, not to think of anything else but a beautiful baby you are going to have.

After two to two and half months, have a repeat Semen analysis from a different center, preferably from an IVF center where an andrologist can examine, as the report is not complete and seems not well analyzed (morphology should have where in sperm the defect is - head, neck or tail or complete sperm- helps in diagnosing cause).

Have abstinence of three to six days and check (after sex, not less than three days, but not more than six days). Before collection tells him to pass urine and wash his penis with water only, no soap - as soap will damage sperms. Collect complete semen, even the initial clear fluid, as it contains the highest concentration of sperm and collect it completely.
