Patient's Query

Hello doctor,

I am a 27 year old female. Actually, my problem is I do not get wet, and I do not experience any sexual feeling whatever I watch or whatever I do (I mean sexual act or masturbation). Why is it so?

Hello,

Welcome to icliniq.com.

I can understand what you are going through at present. Sexuality is such a topic that most of the people especially females do not discuss. Due to the stigma attached, they do not even come out with their sexual problems. So, I would like to appreciate your initiative to come up with your problem.

As you told that you have difficulty in getting wet or stimulated after watching any intimate stimuli and also have difficulty in having sexual activities as you do not get pleasure from them. What you are suffering from is called "female sexual interest/arousal disorder" (FSIAD).FSIAD is characterized by:

  1. Absent or reduced sexual interest in sexual activity.
  2. Absent or reduced erotic thoughts or fantasies.
  3. Absence of or reduced initiation or receptivity to sexual activity.
  4. Absent or reduced sexual excitement/pleasure during sexual activity in all or almost all (75–100%) encounters in contexts identified as problematic or, if the problem is generalized, in all contexts.
  5. Absent or reduced sexual interest/arousal to sexual/erotic cues.
  6. Absent or reduced sensations during sexual activity in all or almost all encounters in contexts identified as problematic or, if the problem is generalized, in all contexts.

It can be caused by endocrine dysfunction, metabolic disturbances, cardiovascular disorders, CNS pathologies, spinal cord injuries, urogenital pathologies, medications, hormonal disturbances, relational issues, emotional disturbances, physical/sexual abuse, and violence. It needs a detailed evaluation to rule out the above-mentioned causes, to plan management for the same. Management includes both pharmacological in the form of hormonal treatment; and psychosocial interventions such as sensate focus therapy, sexual skill training, communication skill training.
