Hello doctor,

I have ED. If I am trying to have intercourse with my partner, I am not able to get it to move an inch. But when I am alone and try to masturbate, it takes a little time but it gets fully erect. I am currently taking no medications. I am 25 years old. This issue started a few years back and started to become worse gradually. At first, it was able to get hard to a certain level but now it feels totally numb. Please help.

I can understand your concern.

According to your complaints, you may be suffering from erectile dysfunction (ED).

Some of the reasons for erectile dysfunction (ED) include the following:

1. Stress due to work or family-related, night duties - plan for regular exercise and yoga.

2. Depression.

3. Varicocele - ultrasound scrotum can help diagnose it.

4. Fear of failure - having one episode of ED once may make you feel you will end up with ED.

5. Low self-esteem (chronic masturbation).

6. Homosexuality - attracted to same-sex.

7. Religious constraints.

8. Smoking and alcohol.

9. Long-term illness - cardiac disease, thyroid disease, kidney disease, or any other disease with long-term medication.

10. Some vitamin deficiency.

11. Negative attitude towards your partner.

12. Hormonal issue – thyroid issue, low testosterone, increased prolactin.

13. Long-term drug usage.

14. Diabetes and hypertension.

You need to check the Doppler penis, USG (ultrasound) pelvis and scrotum, blood pressure, RBS (random blood sugar), Free T3 T4, Testosterone, and TSH (thyroid stimulating hormone). Also, share more details regarding the above reasons so we can work on diagnosis and treatment.

You also need to plan for regular exercise and a proper diet:

1. List of exercises: basic exercises like Kegels, Squats, Yoga, running, or fast walking for 5 minutes.

2. Missionary sex position: Planks, Push-ups, Mountain Climber, Glute Bridges.

3. Standing sex position: Bicep Curls, Pull-ups, Jump Squats.

4. Doggy Style: Jump Squats, Walking Lunges.

5. Cowgirl or the girl on the top move: Glute Bridges, Leg Raises.

6. Food: Strawberry, apple, avocado, sweet potato, sesame seed, almond, garlic, and little chocolate.

For a temporary solution, you can take the tablet Mildfil 5 mg twice a day. However, please consult a physician, discuss with them, and start taking the medications with their consent.
