Can Tezspire treats EGPA effectively?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

A 36-year-old female with severe asthma and newly diagnosed eosinophilic granulomatosis with polyangiitis (EGPA) (p-ANCA positive). Current manifestations include asthma, sinusitis, peripheral eosinophilia (1500 cells/µL), and mono-neuritis multiplex. She has started on Prednisone 40 mg daily and is being considered for Cyclophosphamide. Could Tezspire (Tezepelumab) be considered as part of EGPA management? How would it integrate with immunosuppressive therapy? What monitoring is needed for potential vasculitis flares? Should we modify the dosing schedule based on other immunosuppressive medications?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

To better understand the patient's condition and provide more tailored advice, could you please share the following information:

Recent blood reports-

1. Complete blood count (CBC) with differential.

2. Erythrocyte sedimentation rate (ESR).

3. C-reactive protein (CRP).

4. Immunoglobulin E (IgE) levels.

5. Blood chemistry, including renal and liver function tests.

Pulmonary function tests (PFTs):

  1. Spirometry results: Forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), FEV1/FVC ratio.

  2. Lung volume measurements: Total lung capacity (TLC), residual volume (RV), functional residual capacity (FRC).

Additional information:

  1. Current Body Mass Index (BMI) and weight.

Detailed clinical history, including-

1. Age of onset and duration of asthma and EGPA (eosinophilic granulomatosis with polyangiitis).

2. Previous treatments and responses.

3. Presence of any comorbid conditions (e.g., hypertension, diabetes, cardiovascular disease).

4. History of addiction or substance abuse.

5. Family history of asthma, EGPA, or other autoimmune disorders.

Current medications-

1. List of current medications, including dosages and frequencies.

2. Any recent changes to medications or dosages.

This information will help provide a more comprehensive understanding of the patient's condition and guide further recommendations.

Considering Tezspire for EGPA management-

1. Efficacy in EGPA: Tezspire (Tezepelumab) targets the thymic stromal lymphopoietin (TSLP) pathway, which plays a role in eosinophilic inflammation. Its efficacy in EGPA is being explored.

2. Potential benefits: Tezspire may help reduce eosinophilic inflammation, a key component of EGPA.

Integrating Tezspire with Immunosuppressive therapy-

1. Combination therapy: Tezspire could be considered as an add-on therapy to immunosuppressive agents, such as Cyclophosphamide, to enhance eosinophilic inflammation control.

2. Monitoring and adjustments: Close monitoring of eosinophil counts, clinical symptoms, and potential side effects is crucial. Adjustments to immunosuppressive therapy may be necessary based on the patient's response.

I hope this helps.

Kindly follow up if you have more concerns.

Thank you.
