Hello doctor,

I had unprotected sex two days after my period and took ECP within 24 hours. I have not seen my period yet. What could be the reason? Please help.

A delay of up to one week (7 to 10 days) is generally considered normal if it occurs occasionally, such as once or twice a year, and if there are no underlying medical conditions contributing to irregular cycles.

Common causes of delayed menstruation include:

  1. Pregnancy should be ruled out in sexually active individuals.
  2. Psychological stress affects the hypothalamic-pituitary-ovarian axis and can disrupt hormonal balance.
  3. Thyroid disorders, including hypothyroidism (underactive thyroid) and hyperthyroidism (overactive thyroid), both of which influence menstrual cycle regulation.
  4. Sudden or significant weight loss can lead to hypothalamic amenorrhea due to reduced leptin levels and disruption of gonadotropin-releasing hormone (GnRH) secretion.
  5. Obesity (excessive body weight) affects estrogen metabolism and may lead to hormonal imbalances.
  6. Excessive physical exercise can suppress ovulation due to high cortisol levels and energy deficit.
  7. Use of hormonal contraceptive pills (oral contraceptives), which can cause cycle irregularities due to synthetic hormone effects on endogenous hormone production.
  8. Polycystic ovary syndrome (PCOS) is a condition characterized by multiple ovarian cysts, insulin resistance, hyperandrogenism (excess male hormones), and irregular ovulation.
  9. Endometriosis is a disorder in which endometrial-like tissue grows outside the uterus, using menstrual irregularities and pelvic pain.
  10. Hormonal imbalance triggered by sexual activity, which may cause temporary alterations in menstrual cycle timing due to physical or emotional factors.
  11. If menstruation is delayed, it is advisable to wait for spontaneous onset while maintaining a healthy lifestyle and monitoring any associated symptoms.
  12. If menstruation does not occur and the delay extends beyond 10 days, a urine pregnancy test (home pregnancy test) should be performed to check for possible pregnancy.
  13. If the urine pregnancy test result is negative and menstruation has still not occurred, a blood test for beta-human chorionic gonadotropin (beta-hCG) should be conducted to confirm or exclude early pregnancy with higher accuracy.
  14. If both tests are negative and menstruation remains absent, a visit to the gynecology outpatient department (gynecology OPD) is recommended for a comprehensive evaluation, including hormonal profiling, pelvic ultrasound, and assessment of potential underlying gynecological or systemic conditions.
