What could be the reason for extreme dysmenorrhea?

Hello doctor,
I am a 18-year-old female and I just want to ask what is the reason behind my extreme dysmenorrhea. I suffer every month from severe dysmenorrhea. On a scale of one to ten, I am am having ten or very severe pain. I do not have any other symptoms except for this. It is really painful. My color of the blood is red and I am having a normal flow with regular periods. I am quite underweight and I am tall and thin and I am a virgin. Do I have some kind of cysts or something serious with my reproductive system or nervous system, since the signals are from the pituitary gland? I am tired of being hurt by this every month. Please help me. During pain, I take hot compress and Mefenamic acid. Also, if this is not that serious, what is the reason for this painful menstruation every month? Do I produce too many prostaglandins? Why is it? Do you have any medication advice for me? I am hoping that I will have a useful answer from you. By the way, I want to be like you in the future.

The causes of dysmenorrhea are plenty but the most likely cause is physiological. Perception of pain varies from person to person. It can also be due to the excess production of prostaglandins. This type of physiological dysmenorrhea usually subsides after you are sexually active and at times after a normal childbirth. Till then there are a few tips I can suggest to alleviate the pain.
- Try to keep yourself properly hydrated.
- Keep your bowel regular.
- Try stretching exercises to decrease pelvic congestion.
- I suggest simple painkillers like Mefenamic acid once in a while.
- Take a course of evening primrose oil for six months at 1000 mg per day.
Since you are a virgin you are unlikely to suffer from dysmenorrhea due to infection. There is another cause of dysmenorrhea called endometriosis. It can result in progressive dysmenorrhea. So most likely, you are having physiological dysmenorrhea.
