Hello doctor,

I have large portions of cracked heels on both feet. The skin is really dry and hard, with deep fissures, and I do not know how to fix it. It is not painful at all. My right foot is slightly worse. There is a large protruding hard skin, and I do not know what to do with it. Should I try to cut it off? Or should I seek a specialist who can remove it safely?

I never wear sandals. I only take short showers, not baths. I only use shower gels, not bars of soap. I do not usually add moisturizing cream to my feet, but I have tried to do so recently, but it does not seem to make any difference. I live in an area where the water is very hard.

Please advise on the following:

1. What to do in the short term to remove the existing hard skin from my feet?

2. Is there any possible treatment I can use to soften the dry skin that cannot be removed?

3. What are the best moisturizing creams or oils to prevent it from happening?

4. Is there any long-term strategy to prevent it from happening?

I am a 41-year-old male. The moisturizing creams tried in the past on my feet are nivea body lotion, rich nourishing intensive moisturizing care, and vaseline intensive care essential healing.

I read your query and can understand your concern. I have gone through the history and images provided by your end (attachments removed to protect the patient's identity).

It is plantar keratoderma which is extreme dryness and hardening of skin along with cracking of skin on the sole. They appear due to multiple factors, including genetic susceptibility, wherein there is a similar condition that runs within family and gets transmitted genetically due to climate conditions, dietary factors, use of harsh soaps or excess washing of the area, and as you have already mentioned the type of water as well. They may be managed but may recur if the medications are used consistently due to changes in the skin depending on the various external and internal factors.

My advice :

1. Soak your feet daily in warm salt water for 15 minutes in the morning and gently scrubbing the area with a pumice stone.

2. Aquaphor healing ointment to be used in the morning daily in a generous amount.

3. Urea 40% cream with Salicylic Acid and Hyaluronic Acid to be applied at night on the affected areas. They may be continued till the area softens.

4. Avoid shower gels and soaps.

5. Oilatum bath formula of three full bottle cap in 1 liter of water may be used for soaking feet in them for five minutes and washing the area as well at night before applying the cream as mentioned above.

6. Avoid oily and fried food, sweets, and red meat. Make sure to include more fish and at least 12 glasses of water a day for hydration.

7. Capsule Omega-3 once daily after breakfast to be consumed for two months for moisturization.

8. Capsule Evion 400 or vitamin E capsule 400 mg to be consumed at night after dinner for two months for hydration.

9. After using Urea cream at night, wear comfortable socks after 20 minutes of applying the cream to ensure they are well absorbed.

Continue these medications till the area becomes softer.

However, please consult a physician, discuss with them and start taking the medications with their consent.

This is an excellent and detailed answer. Thank you. Just one question: In items 5, 7, and 8, you use the words "cap ful" or "cap". What is a "cap"? I don't really understand what that word means. Please clarify. Thanks.

Cap full that I have mentioned means the cap of the bottle which is used to close it. The emollient is measured based on the cap of the bottle. Therefore, 3 cap full means the cap used to measure the product thrice. Hope I have cleared your doubt. Have a great day ahead.
