Hello doctor,

I am a 17-year-old female, weighing 115 pounds and standing 5 feet 3 inches tall. Over the past week, I only got around four hours of sleep each night due to finals week, which caused me a lot of stress. I also barely drank any water during this time. One morning, I woke up with a strange feeling in my throat, as though I was short of breath, and coughing did not help. The next day, I woke up with a runny nose, a headache, and a fever, and felt very weak. I had to babysit for four hours, so I took two Tylenol before I left. While babysitting, I only had a fever and kept experiencing hot and cold flashes. When I returned home after babysitting, I felt freezing due to the fever. I drank vitamin C water and immediately took a three-hour nap. The rest of the night, I alternated between feeling very hot and cold, with a fever of 102°F.

The following morning, I woke up with an extremely bad headache, the same dry throat and shortness of breath, a runny nose, and a fever. I was also extremely sore. I spent the entire day in bed, too sick to speak without coughing, too congested to sit up due to a runny nose, and I could not manage my fever, so I had half of my body under the covers and the other half exposed. I took Tylenol and the children’s Claritin.

The next day, I was finally able to function and took a shower. However, throughout the day, I have been unable to breathe through my nose because it is so stuffed up. I also cough about every five minutes, and when I cough, I feel like I cannot catch my breath. What could all of this mean? My school has also had a few cases of whooping cough circulating for a few months, and I suspect my lack of sleep and dehydration during the stressful finals week did not help.

Based on your symptoms, I believe you may have developed an upper respiratory tract infection, which could be caused by either a bacterial or viral infection. Have you had any recent contact with someone who has the flu? Do you stay in a cold environment or drink cold water? Are you experiencing any ear discharge, eye discharge, or sore throat?

I suggest you get a PCR test (polymerase chain reaction) for Haemophilus influenzae and let me know the result. In the meantime, please take the following:

  1. A tablet of Acyclovir 800mg orally twice a day for one week.

  2. Two tablets of Vitamin C 1000iu orally three times a day for two weeks.

  3. Drink plenty of water.
