Hello doctor,

I am a 22-year-old female suffering from polycystic ovarian syndrome. I was taking medications for it, but I was still having irregular periods, so I stopped taking them. Now that I have been married for the last two months, I am afraid that I might not be able to find out if I got pregnant because of my irregular periods.

Please recommend me appropriate birth control pills for protection from pregnancy.

I read your query and understood your concern.

Firstly, I would like to inform you that polycystic ovary syndrome (PCOS) is not a disease but an imbalance of hormones. For a suitable contraceptive medication, I suggest you to take a tablet containing the below combinations.

  1. Desogestrel 0.15 mg + Ethinylestradiol 0.03 mg
  2. Ethinyl Estradiol 0.05 mg + Norgestrel 0.5 mg
  3. Ethinyl Estradiol 0.05 mg + Levonorgestrel 0.25 mg
  4. Ethinyl Estradiol 0.02 mg + Desogestrel 0.15 mg

All these are 21-day pill packs. I suggest you take one tablet of any one of the pills two times a day for ten days and throw away the 21st pill. You will get your period within seven to ten days. From the first day of the period take the same tablet one-time for 21 days; for the remaining seven days, do not take any pills; then, from the eighth day, start taking the tablet once daily the same way. Do not wait for the periods. Repeat the same cycle till you want contraception. Do not skip or forget to take a tablet on any day in between, as it may cause bleeding and hamper its effectiveness. So the method is to take one tablet for the first 21 days, skip seven days, and start a new pack on the eighth day. I suggest you consult a specialist, talk with them, and take the medications with their consent.

Regarding PCOS, It is usually associated with obesity, irregular cycles, no periods for a few months, and later heavy periods with blood clots, thyroid hormone imbalance, and prolactin problems indirectly. PCOS may not allow ovulation to occur many times, so you may get fertility issues. It also increases the chances of early pregnancy miscarriages. For PCOS, I suggest you follow the instructions mentioned below,

Maintaining weight - Obesity can be a factor causing PCOS. You can check your ideal weight with this formula. Weight (in kgs) = height (in cms) -100. For example, If your height is 160 cm, 160 - 100 = around 132.27 to 143.3 pounds is your expected weight. If you are overweight, reducing weight to the normal range can help to get regular periods without the use of pills. In addition, you can follow the instructions mentioned below for weight reduction.

  1. Eat healthy food.
  2. Avoid junk food.
  3. Consume low calories food with high protein content.
  4. Perform regular exercise, aerobics, yoga, and walking.

As soon as you reduce at least 11 pounds, you will notice positive changes.
