Why does a persistent velopharyngeal gap remain after surgery?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My daughter has nasal speech and only a cleft palate with no cleft lip. The back of her throat is not closing completely. She had undergone Furlow Z-plasty, but still, there was a gap.

Please help.

Hello,

Welcome to icliniq.com.

Can you tell me when she underwent this Z-plasty procedure? Usually, the healing period and gap closure depend on the soft palate's length. The next step will be another surgical procedure, depending on when you got this done.

It is called pharyngeal flap surgery or sphincter pharyngoplasty. It is recommended only when there is no improvement in speech or gap closure is insufficient.

I hope this helps.

Thank you.
