Hello doctor,
I am 62 years old. I have been on kidney dialysis for the past two years. A few weeks ago I developed CKD-MBD. I am taking a drug to lower my phosphorus levels (8.5), D3, and niacin as well as the normal B12, calcium, and vitamins. How long will I have to deal with the constant pain in my pelvis bone and joints? Will the pain subside as calcium is returned to the bones and my blood calcium levels are raised? What painkiller do you recommend?

I understand your concern regarding your CKD (chronic kidney disease)and MBD (mineral bone disorder) status. Your calcium is low and phosphorus is high, but this does not explain your PTH (parathyroid hormone) level, it should have been high but it is not. Since when are you taking phosphorus and calcium tablets? For pain, you may take a tablet of Paracetamol (Panadol, Calpol, Alvedon) and a tablet of Tramadol (2-dimethylamino methyl, 1-3-methoxyphenyl cyclohexanol) combination. Along with medications, do exercise and strength training twice a week.

You should continue the test for PTH (parathyroid hormone) levels after three months and calcium, and phosphorus levels after a month. For pain take the medication I advised. Continue exercise and strength training to prevent your bones from getting weak. Avoid a non-vegetarian diet it may aggravate bone disease by extracting out calcium from bones. Do not worry about the protein, take it in the supplement powder form recommended for dialysis patients. Eat more amount of fruits and vegetables (only those recommended to you).
