Hi doctor,

My 4-year-old boy has a fever. The child coughed for two weeks. Chronic bronchitis was diagnosed. I gave Amoxicillin for five days. After that, he got cold again. Reddened skin of the eyes, swollen eyelids, red pharynx and corners of the mouth, swollen tonsils with purulent plaque, and enlarged lymph nodes in the neck and forearms were found. Rough vesicular breathing is heard in the lungs. There are no skin rashes. The edge of the liver was grasped. The spleen and liver were enlarged. There was slight leukocytosis. What is the possible diagnosis? Please help.

I can understand your concern.

As per the information provided, looking at all the signs and symptoms, eye changes, cervical lymphadenopathy, eyelid swelling, redness over corners of the mouth, along with low platelet count, I will still put strep on the top of my differential diagnosis and then to go and investigate for EBV (Epstein-Barr virus) infection, and dengue. On the bottom of the differential diagnosis, I will keep the incomplete kawasaki.

  1. Is the boy clinically active and feeding well?

  2. Is he passing urine well?

  3. What is the blood pressure?

Look for any skin peeling over fingers, groin, and perianal region. I will start again on an empirical antibiotic after the collection of all the samples for investigation and wait for the report to decide on the next step. Keep us updated.
