Hello doctor,

I have been experiencing chest pain for the last one year. The echo was taken three times; an angiogram was normal, troponin I was normal, but the ECG was abnormal. The doctor said there was nothing in my heart, but why did I have chest pain along with neck, shoulder, and back pain? Why are ECG reports always abnormal? Please help.

I can understand your concern, but I suggest it is likely a spine problem like spondylosis, disc problem, or cervical spine. Here, you get pain and other symptoms due to the compression of nerves between the spine. AnAngiogram is a definite test that reliably excludes significant problems. Also, heart disease is not at all expected in you as heart symptoms would be very typical, occurring on exertion and relieved with rest unless it is a heart attack, which is highly unlikely as tests are negative.

I have not seen your ECG (electrocardiogram), but it may be a normal variation, which is quite possible. You can upload your ECG report for a better opinion. I also suggest you undergo an exercise stress test, Vitamin D level, X-ray of cervical spine AP (anteroposterior) / lateral view, and further may need MRI (magnetic resonance imaging) scans if x-rays are not informative. Do you have any history of trauma to your spine? Does this pain increase with neck movement? Is it associated with tingling, numbness, needles, and pins sensation? I suggest you take the following treatment.

Do Physiotherapy and neck exercises, cervical collars, andmedicine. Take tablet containing vitamin B complex once daily, which is a nerve vitamin. Apply Diclofenac (non-steroidal anti-inflammatory drug) gel for local application. After confirming the diagnosis, we can prescribe you a nerve stabilizer like tablet Pregabalin 75 milligrams (gamma-aminobutyric acid), which will stabilize the nerve and reduce pain. You can visit an Orthopedician for all these investigations.
