How to cope with worries surrounding cesarean birth?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My wife came to know that she is pregnant. It is going to be our second child. Our first child is 3 years and 8 months old now. During the first pregnancy, everything was fine (except she got her IgM test positive and the doctor advised Rovamycin Forte at that time).

The first delivery was cesarean, and everything was fine. One woman in our city was diagnosed with a hole in her heart during the day of her cesarean, and because of this, she died after a day of giving birth to a baby girl. It has become a fear for me.

Does my wife require any tests to diagnose whether she is having any of such things?

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

As there was a safe cesarean section previously so it is unlikely that she has any cardiac issues, which would have manifested during her previous pregnancy. However, as she is now pregnant again, so complete antenatal check should be done again, including Torch Titre, as she was IGM positive in her previous pregnancy and had to be started on Rovamycin Forte. Also, maternal cardiac echocardiography should be done (to check the cardiac status of the mother).

Do follow up for further queries.

Hope this helps.

Thanks and take care.

Patient's Query

Hello doctor,

Thank you for your reply.

We checked with a local gynecologist, and he said that no pregnancy is visible on ultrasound (he did the ultrasound in his own clinic, so no report is there). But the urine test is positive, and the pregnancy test kit is positive.

Is there anything serious like cancer?

As I saw on the internet that this type of situation is possible when there is an unusual ovarian mass.

Please help us, we are worried. I am attaching the prescription.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

As of date, she is just 5 weeks and 5 days of gestation (calculated from LMP), so there is a chance that the gestational sac may not be seen so early. So wait for the next 10 days and repeat the ultrasound again to check for an intrauterine or extrauterine (ectopic pregnancy) gestational sac, fetal pole, and fetal heart rate.

The presence of pregnancy outside the uterus is termed ectopic pregnancy, and it is an emergency, so wait for 10 days.

Hope this helps.

Thanks, and take care.
