Why does my penis hurt though all STD tests are negative?

Hello doctor,

I had sex with a lady and after three days my penis gets pain a lot in the night. I cannot sleep much in pain. After one week I checked with the hospital, they give me antibiotic medicine but there is no relief. I have done my herpes simplex virus (HSV) 1, HSV 2, venereal disease research laboratory (VDRL), and human immunodeficiency virus (HIV) test. All test are negative. Now one year has gone, still, I have pain little inside the urine pipe when I go for urine and on the tip of my penis a red dot appears. On applying Candid cream the red dot goes away but when I stop applying the cream again that red dot appears. I am also taking AF-150. I am sending you my picture. Please help.

Welcome to icliniq.com. I have gone through your history and reviewed the attached photograph. (attachment removed to protect patient identity). According to me, there are two possibilities, one is candidal balanoposthitis and the other is herpes genitalis. Since you have mentioned that your HSV 1 and 2 tests (herpes simplex virus) were negative then chances of candida infection is more. But I would recommend you to repeat HSV 1 and 2 tests again to rule out herpes infection. As far as treatment is concerned, wash the affected area with water only and keep it as dry as possible. Apply Sertaconazole cream on the affected area twice daily for at least two weeks and take tablet Fluconazole 200 mg twice weekly for two weeks. You can revert back to me after two weeks. The probable causes are candida infection, herpes infection. Investigations to be done are enzyme-linked immunosorbent assay (ELISA) for herpes simplex virus (HSV) 1 and 2. The probable diagnosis is candidal balanoposthitis. Treatment plan and preventive measures are as mentioned above.

I got this infection last March, after three months I tested HSV 1 IgG and HSV 2 IgG. After that I was checked by a skin doctor he gave me Virovir 250 for five days, but my problem has not solved. Now I go to the lab and they ask me which test I want like 4 tests of HSV namely HSV 1 IgG, HSV 1 IgM and HSV 2 IgG HSV 2 IgM. I thought of testing HSV 1 and HSV 2 IgG. What is HSV 1 IgM and HSV 2 IgM? I need to test all four that and how period this virus appears in the test? I send you some of my pictures. That yellow dot in the picture just appears before two days and that red dot mostly appears when I stop using Candid cream.

The yellow and red dots appear as pustules of candida infection. You use the medicines that I have already prescribed. As fas as HSV tests are concerned go for all the four as HSV 1 IgM and HSV 2 IgM indicate recent infection whereas HSV 1 IgG and HSV 2 IgG indicate past infection.
