Hello doctor,
I have been experiencing severe burning during urination and have noticed yellowish discharge for the past ten days. My urine analysis revealed 8-10 pus cells and 4-6 epithelial cells, with cloudy urine. A urine culture showed occasional pus and epithelial cells but no growth of bacteria. I initially consulted a gynecologist who diagnosed me with fungal vaginitis and prescribed Zenflox for a UTI, which I have been taking for 5 days, but I have not seen any improvement.
I am concerned about the possibility of a sexually transmitted disease, bladder or kidney infections, or even tumors. I have a family history of cancer, which makes me worried about ovarian cancer. I had a pap smear two years ago, which came back normal, and I was prescribed Cefoderm XP 325 for 7 days at that time.
Could you please advise whether I should see a urologist, gynecologist, or undergo any specific tests or scans to address these concerns?

The presentation of urinary infection can often be similar to that of vaginal infection, and treating it solely with antibiotics may not fully resolve the issue.
Vaginal infections can be transmitted during sexual intercourse, with both partners needing treatment simultaneously to prevent reinfection. Symptoms may include white, yellow, or grayish discharge, along with or without itching, a foul odor, and pain during sex due to dryness (due to reduced lubrication). There can also be discomfort toward the end of intercourse when the fully erect penis touches the innermost parts of the vagina due to inflammation of the structures around the uterus. This can cause pain radiating to the back or thighs.
Infections can also affect bowel movements and may exhibit symptoms of gastritis. Menstrual bleeding may become thicker, darker, contain clots or fleshy masses, and may have a bad odor. There may be more days of spotting after the period as well. In cases of infection within the uterus, increased blood flow due to inflammation of the inner lining can lead to the formation of a thick, doughy or fleshy mass. Incomplete healing of the lining can result in ongoing spotting or blood-stained discharge. Throbbing pain may occur before and after periods. In most cases, the infection is mixed, involving a variety of aerobic and anaerobic bacteria, some of which may not be detectable through culture. Treating one type of infection may lead to the growth of another, and incomplete treatment can result in regrowth.
Before starting treatment, it is important to confirm that you are not pregnant.
I would suggest both you and your partner take the following medications:
Take all of these on the same day, and both partners should follow this regimen. I would also suggest you perform vaginal washes with mild vaginal cleanser twice daily, and use a vaginal pessary like Capsule Clingen Forte (Clindamycin, Tinidazole and Clotrimazole) once at night for 5 nights. Your partner can apply any antibiotic and antifungal cream to the penis twice a day after washing for 7 days.
Consult your specialist doctor, talk with them, and then take the medications as per their advice.
Avoid sexual intercourse until the infection is fully resolved, as delaying treatment may push the infection inward and affect future fertility for both you and your partner. Please note that Zenflox (Ofloxacin and Ornidazole) is primarily a urinary antibiotic and may not significantly address a vaginal infection. Also, a urine analysis showing 8-10 pus cells is not typically indicative of a significant urinary infection. I suggest having fasting and post-prandial blood sugar tests done. If either you or your partner has diabetes, it is essential to manage it effectively, as it can contribute to this problem. Concerning ovarian cancer, it usually presents with no symptoms and is often an incidental finding. A scan is a straightforward way to detect it.
