Patient's Query
Hi doctor,
I am a 65-year-old male. I weigh 147.71 Lbs while my height is 5.61 feet. About 14 years ago, I first went for a master check-up without symptoms. The result showed elevated 2-hour PPBG while FBG and A1c were normal. The consultant physician advised me to eat less at a time, increase the number of feeds in a day, and advised me to check my BG level once every three months.
Accordingly, I have done the tests both at home and in laboratories every three months since then. My FBG is always normal at 80 mg/dL to 85 mg/dL, and it rarely crosses 100 mg/dL, but within 105 mg/dL. My A1c is also quite normal, 4.2 % to 5.4 % since the first check-up.
However, my PP after two hours is always in the range of 150 mg/dL to 176 mg/dL. However, when I take reading after three hours of ingestion, the PP value is in the normal range of 120 mg/dL to 140 mg/dL. I do not take any medicine or insulin. I do not have symptoms like frequent urination or too much thirst, or dizziness.
Nevertheless, I suddenly developed tinnitus in both ears a few months ago. An MRI was taken and found normal. Hearing is fine, and Minner's disease was ruled out. The volume in the left ear is more than in the right ear (the left is 6/10, whereas in the right it is 3/10). It is the insect cricket sound like.
Now, last week, while I was working on my computer sitting on my usual chair, I developed a catching sensation in both my calves all of a sudden. A bit of burning sensation in the calves was also felt. It still continues.
Yet, I walk without any difficulty and do the routine. No tickling or pinning sensation. I am in a remote village, spending my post-retirement with family. My wife, a clinical biochemist, advised me to rule out peripheral neuropathy as my two-hour PP test is always in the range of 150 mg/dL to 176 mg/dl through online consultation for relevant medical tests to be done in this regard.
I also have a mild pulling sensation at the back of my right thigh. I, hence, request you to suggest tests to undergo to find out the cause of the problem. Yesterday and today, I tested my BG levels, and I attached the results for you to check. My BP is always 110/70 mmHg to 120/80 mmHg.
I normally walk 1.25 miles a day. I do not smoke, have no alcoholic drinks whatsoever, and I am a vegetarian. The lipid test done a month ago is quite normal. It may be helpful to mention that my total cholesterol and LDL counts have always been low, 120 mg/dL and 45 mg/dL respectively, for several years now.
I await your advice.
Please help.
Hi,
Welcome to icliniq.com.
I thoroughly read your query and understand your concern.
Yours is impaired post-prandial glucose, occasionally having impaired fasting glucose also. It is prediabetes. People with prediabetes can have neuropathy. I suggest you test for CBP (complete blood picture), vitamin D, vitamin B12, serum calcium levels, and NCS (nerve conduction studies). Do you smoke (you said no, but did you stop or never smoked)?
Please revert with answers.
Regards.
