Can Doxycycline cause a burning sensation in the urethra?

Hello doctor,

I had unprotected sex with my ex-girlfriend. I became paranoid about catching a sexually transmitted disease like chlamydia as she has had a couple of partners. So I took Doxycycline. After completing the Doxycycline, I started having to burning sensation in my urethra after peeing. I went to a urologist, and he checked my urine for bacteria associated with STDs, but he found nothing. No protein, sugar, or bacteria in my urine. He sent my sample off for deeper analysis, but they still found nothing. I also did a blood test for HIV and syphilis just over a month after exposure, and all came back negative.

Here are the answers to your questions.

  1. I am 49 years old.
  2. I am not diabetic or hypertensive.
  3. I have had no surgeries in the past.
  4. I do not have any unusual urgency to urinate, increased frequency, or feeling of incomplete emptying.
  5. I drink a lot of water every day, so I go quite regularly anyway. Overall, no change in my urination cycle.
  6. I have had no visible blood in my urine. I ejaculated last week then I felt a slight cramp in my testicles because I did not have sex for a while or masturbated, as I was trying to not irritate my urethra, but after ejaculating the burning increased, so that is when I went to the doctor and was prescribed penicillin.
  7. Before this period I masturbated about 3 to 4 times a week, but since this issue, I have not done it like before to not irritate my urethra.
  8. I have have no lesions on my penis. I took a day's break from the medicine.

I have about four more days of the medicine to go. Should I continue taking it?

To me, it appears that you have developed urethritis (inflammation of the urethral lining) and you may have some prostatitis as well.

Going by your age, there may be a component of an enlarged prostate as well.

Here is what I suggest you to do:

  1. Continue drinking around 3 liters of water per day.
  2. Finish the course of your penicillin as stopping midway is worse than taking it for unindicated reasons.
  3. Visit your urologist and ask for a uroflowmetry test. This will determine how well you are passing urine.
  4. Take over-the-counter anti-inflammatory drugs twice a day for 5 to 7 days.
  5. Ask a urologist to prescribe you an alpha blocker, such as a capsule, of tamsulosin, 0.4 mg at night before bed for a month.
  6. Once the irritation has resolved, continue to masturbate, at least 2 to 3 times a week.
  7. Continue drinking, cranberry juice or lemonade, as that will also help with the burning sensation or irritation.
  8. If you follow the above instructions, I believe you may get significant improvement in a week to 10 days time.
