I had a bump on my penis and I got a scab after healing. It has been three weeks. I had a fever for a couple of days. Now, it looks more like a rash. Kindly suggest.

Please mention whether you have a previous history of genital herpes or any recurrent episode. Please mention whether you had any injuries in the past. Please mention whether you are sexually active or had any unprotected sex in the last three months. Please mention whether you have diabetes.

I do not have any pain or tingling sensation or burning sensation or itching on the lesion. I never had any genital herpes or injury in the past. I am sexually active and it showed up after a day of sex. I do not have diabetes.

It seems to be dwarf chancroid caused by Haemophilus ducreyi. It is transmitted through sexual intercourse. It has an incubation period of three to five days. Clinically, it presents as a yellow necrotic, shallow non-indurated ulcer with a ragged outline. Doing saline compresses two to three times a day helps a lot. You can take tablet Ciprofloxacin 500 mg orally in a single dose or take tablet Doxycycline 200 mg a day for seven days. You can use Vidarabine ointment or T-bact (Mupirocin ointment) twice daily in the morning and evening for ten days. Please mention whether you had any pre-urethral discharge or mucoid or mucopurulent discharge. Please mention whether you had any involvement of groin lymph nodes. Please mention you had any fever and please mention whether you have done any serological tests.

I do not have any urethral discharge. When I wake up in the morning, I can see something sticky in my underwear. It seems to be a mucoid or mucopurulent discharge. My eye seems to be crusty. I do not have any involvement of groin lymph nodes. I got a fever for few days. I have done serologist tests. I did my STD test two days back and the results came back negative except for herpes and my doctor advised me to do a culture test. I have attached picture showing groin lymph node involvement.

Regional lymphadenopathy occurs in 50 percent of patients having chancroid. Papular chancroid, mixed chancroid, transient, follicular, and dwarf chancroid are some variants of ulcers. You can take tablet Azithromycin 1 g orally as a single dose or take Ciprofloxacin 500 mg twice daily for three days or take tablet Doxycycline 100 mg twice daily after food for seven days. Avoid having sex until your lesion heals. You can use barriers and have safe sex. You can use a mild cleanser like Episoft cleanser to clean genitals. Avoid scrubbing or scratching of lesions as it can cause secondary bacterial infection. Your attached picture shows folliculitis with infected hair follicles (attachment removed to protect patient identity). You can use Fucidin cream (Fusidic acid) twice daily for seven days. Please mention whether you have pain on it and whether you ruled out it as a syphilitic chancre. Please mention whether you have a negative result for VDRL (venereal disease research laboratory test), RPR (rapid plasma reagin), and herpes genitals. Please mention whether you have done herpes simplex virus (HSV 1 and 2) IgG (immunoglobulin G) and IgM (immunoglobulin M) tests. Please mention whether you have done a serological assay. Please mention whether you have done ELISA (enzyme-linked immunosorbent assay) for HIV (human immunodeficiency virus) or HBsAg (surface antigen of hepatitis B virus) tests. Please mention whether a gram stain smear was prepared from the base of the ulcer. Please mention whether you have done any bacterial culture. Please mention whether it looks like LGV (lymphogranuloma venereum). Please mention whether have difficulty in retracting foreskin and difficulty in passing urine. Please mention whether your ulcer bleeds and causes pain. Please mention whether you are diabetic. Please mention whether you have done any urethral swabs. If yes, then kindly attach your reports.
