How to treat a cluster of bug-bite-like lesions on the back that pain when pressed?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 38-year-old otherwise healthy male. I woke up with a cluster of bug-bite-like lesions on my back. I only have pain when pressing but no itchiness. It got worse the next day. I noticed a mildly painful swollen lymph node under my axilla. I started to use Fucicort cream twice a day as per an online doctor's advice who thought this was an allergy. But the lesson is getting worse. It is becoming reddish, darker, and elevated from the surface. I do not have any systemic symptoms. Again, the pain is felt only when it is pressed. Kindly help.

Hello,

Welcome to icliniq.com.

I have gone through your history. In my opinion, it is insect bite contact dermatitis or you can say the allergic reaction to the insect sting. It is fine to apply Fucicort cream (Fusidic acid and Betamethasone) two to three times a day. Sometimes the reaction get worsen before subsiding. In a day or two it will form a pinpoint lesion and at that point, pus will discharge then switch to Fudic cream (Fusidic acid) (and stop using Fucicort. In addition to this, you also need to take some anti-allergy to stop the allergic response. I suggest you take a tablet of Cetrizine 10 mg at night for five days. As you say lymph node has also enlarged take a tablet of Brufen (Ibuprofen) 400 mg two times a day and a capsule of Amoxil (Amoxycillin) 500 mg three times a day.

Thank you.
