Hello doctor,

I am a 43-year-old male in good health and workout for four to five days a week. However, for the past year, I have had mild bruising on my skin and some red spots in my mouth, which is petechia. I had my blood test done a few years back, and it was all good. I consulted a doctor last week and told him about the bruising and did tests like PT and PTT tests along with antibody tests which were normal, except for my PT, which was prolonged. The doctor referred me to a hematologist. I think I have some kind of bleeding disorder, but I do not have any other symptoms. Kindly help.

Usually, bruising like bleeding can be expected due to platelet problems or vascular causes like vasculitis. Sometimes vitamin C deficiency also can lead vasculitis like problem. But as you have mentioned that your PT (prothrombin test) has been prolonged, it could be some form of coagulation problem of extrinsic factor pathway coagulation disorder. I want to check all your reports. So kindly attach your reports so I can check them and guide you accordingly. The hematologist will physically examine you and will advise you to investigate with a coagulation profile as well. So provide your reports, and I will check them.

I do not have my reports currently. I did talk to someone who said it sounds like a factor VII defect, but I am still determining exactly what that is, and there seems to be conflicting information about the cause of it through my research. Kindly help.

Prolonged PT (prothrombin time) suggests a defect in extrinsic or common pathway coagulation factor problems. For better understanding, the coagulation hemostatic pathway can be divided into intrinsic and extrinsic coagulation pathways. The extrinsic pathway problem leads to PT elevation. Intrinsic pathway problem leads to APTT (activated partial thromboplastin clotting time) prolongation. Factor 7 is included in the extrinsic coagulation pathway. So its deficiency can lead to PT prolongation. In coagulation assay profile reports, its estimation will be done. Liver problems and vitamin K deficiency also can lead to prolonged PT. But your liver profile is normal, so it can be excluded.

In case of a platelet function problem, the platelet count can be normal, and the bleeding time test can be raised. So bleeding time investigation also needs to be done. Your hematologist will examine you and will try to find out any hemostatic disease if present through a platelet function analysis report and coagulation profile investigation.

I understand how a bleeding test is done, but I never had prolonged bleeding after a cut. When I bleed, it tends to stop pretty quickly. The only time I had excessive bleeding was when I had a gum graft this summer, and at one of the surgical sites, it bled pretty bad the day after surgery, and I developed a liver clot that fell out.

My mouth always cut pretty easily and bruised if I bit my cheek. For years I have had red skin with small red spots on my arms that would bleed if I scratched them. My blood pressure is 130/80 mm of Hg.

You have had mild bruises for one year. Hence above mentioned investigation is needed to exclude any possible hemostatic problem. Moreover, your PT has been prolonged. So, am I suffering from some coagulation deficiency like factor seven or a platelet issue? I had an indigestion issue with milk during childhood. I had this issue a few years back. That year summer, I got ill for about two weeks, and I had a fever, body aches, and chills. Then I had stomach cramps, bad diarrhea, the feeling I had to vomit, and intense headaches. My wife had the same thing. Then one month after, I had my wellness exam, and it came back mildly anemic with elevated liver enzymes and platelets of 121. I also had a positive fecal occult test during that time, but I have since done two at-home tests, which have not been positive. I have had three exams since then, and all my results are normal.

At that time, you could have had a viral stomach infection like gastroenteritis which can present with such symptoms. In addition, stool occult blood tests can be positive at that time due to enteritis. So, the platelet function problem or coagulation factor assay should be ruled out first. A bleeding test investigation is also needed. Blood vessel problems like vasculitis also can lead to easy bruises and petechiae. But it should be thought at last if no cause is evident. Can this condition be inherited or acquired? I did not have any symptoms for one year. I still have the energy to work full-time, care for two kids, and work out five days a week. My appetite is still good; I have not lost weight; I have gained weight.

Mostly acquired but can be inherited as well. Sometimes the expression of inherited problems occurs late in life as well. You have gained weight due to anxiety issues probably. Moreover, in a few patients, Lexapro seems to be causative for a mild increase in weight, so that it can lead to mild weight gain. Kindly revert.

I noticed that during my gum graft. I had bled severely and thought I might have a bleeding disorder, especially with my liver clot at one of the surgical sites. I also noticed my gums bled when I brushed and flossed.
