What are the safe triple-negative breast cancer treatments?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am reaching out regarding my mother, who is 68 and was recently diagnosed with triple-negative metastatic breast cancer (TNBC).

The cancer was found after she developed persistent coughing, and a PET scan revealed lung metastases along with a large primary breast tumor.

She is currently undergoing chemotherapy with Carboplatin and Paclitaxel, but after two cycles, her blood work shows a drop in her hemoglobin and white blood cell count, along with severe fatigue and occasional shortness of breath.

Her oncologist mentioned immunotherapy as an option, but we are concerned about the potential side effects and whether her age might limit its effectiveness.

What other treatments or supportive care options could help manage her symptoms and improve her quality of life, especially with such an aggressive cancer type?

Please help.

Hello,

Welcome to icliniq.com.

I went through your query and understand your concern.

Triple-negative breast cancer (TNBC) is aggressive, and your mother’s anemia with hemoglobin 8.9 g/dL (grams per deciliter), neutropenia, fatigue, and shortness of breath are likely chemotherapy-related.

These issues can be addressed, and to manage side effects, I suggest the following:

  1. For anemia, erythropoiesis-stimulating agents (ESAs) or, if needed, blood transfusions to improve hemoglobin and energy levels, and iron supplements (oral or IV) if iron deficiency is present.
  2. For neutropenia, granulocyte-colony stimulating factors (G-CSFs) boost white blood cells and reduce infection risk, and prophylactic antibiotics or antifungals are used if neutrophil counts are critically low.
  3. Fatigue and breathlessness can be managed with light physical activity, as tolerated, for energy. Oxygen therapy may help if oxygen saturation is low.

A few other therapies that can help are:

  1. Immunotherapy can be considered in combination with chemotherapy, considering her age and potential side effects. A specialist consultation can guide this decision.
  2. Targeted therapies such as PARP (poly (ADP-ribose) polymerase) inhibitors are used if BRCA mutations are identified.
  3. Radiation therapy or symptomatic lung metastases to improve breathing and reduce cough.
  4. If clinical trials are considered, then explore novel therapies for TNBC that may be effective with fewer side effects.

Supportive care may include psychological support and counseling to address emotional and physical challenges. Early palliative care involvement can enhance her quality of life.

Let us monitor her closely and adjust her treatment plan to prioritize both effectiveness and comfort.

I hope this helps.

Thank you.
