Hello doctor,

I have random large holes on my nose that look like blackheads, but nothing is inside. Also, rock hard pimple that has been on my nose for over a year. What is it? It is so embarrassing. Kindly help.

I have seen your query and examined the attached picture (attachment removed to protect the patient's identity). From your descriptions, you have developed pustular acne with blackheads, which means you have grade 2 or 3 acne. You can use topical 1 % Clindamycin gel in the morning and use 3.5 % Microbenz gel (Benzoyl peroxide) at night, and take a tablet of Doxycycline 100 mg once daily for four weeks after consulting your general practitioner or nearby dermatologist.
