Hello doctor,

I am a 29-year-old female. I have norovirus while undergoing bipolar treatment, and my lithium levels are unstable because I can not keep my medication down. Should I switch to injections? I am experiencing manic symptoms but am too weak to move, is this a paradox? My sodium level is 131, and confusion is starting, what is causing this? Is it safe to stop mood stabilizers temporarily? How can we prevent a full episode?

Norovirus is causing severe vomiting, leading to impaired lithium absorption and fluctuating sodium levels. Lithium loss due to dehydration and poor intake can trigger manic symptoms, while low sodium levels (131 mmol/L) and lithium fluctuations may contribute to confusion.

Abruptly stopping lithium can worsen mania, but continuing it without proper absorption and hydration poses risks. Transitioning to an injectable mood stabilizer, such as Valproate or an antipsychotic, may be necessary to prevent a full episode.
