Hi doctor,

I have been experiencing ongoing vertigo for seven years. I have seen an ENT Specialist about the issue and he had diagnosed me with benign paroxysmal positional vertigo (BPPV). He originally did an Epley's maneuver, hoping the dislodged crystals in the ear would go back to its original location. However, after every ENT visit he involved me doing the Dix-Hallpike Maneuver instead. I am concerned where the Epley's maneuver should be administered instead. Also, I had an MRI of the head done and the results came back normal. The neurologist that I also saw mentioned that the condition is definitely inner ear related. I am still having trouble with feeling as though I am constantly moving when I am still. I definitely feel it on my left side and I am currently exercising and doing yoga to help with the symptoms. I would appreciate any feedback, thank you.

I read your query and understand your concern. Dix Hallpike maneuver is a test to diagnose posterior canal benign paroxysmal positional vertigo (BPPV). Whereas the Epleys are corrective measures for it, repositioning maneuver. Once Epleys is done we repeat Dix to check if the debris has been repositioned and vertigo is stopped. If the ENT is repeating Dix in every follow up visit probably the Epleys did not work. Has he put you on any medication ? There are some vestibular exercises which can be done by you at home which will reduce the attacks of vertigo. Has your doctor mentioned those to you ? There are Brandt Daroff exercises as well which you can do at home under supervision. In some individuals Epley's fails. In them we institute medicines and provide vestibular exercises.
