How to perform wound care for stage 4 pressure ulcer?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

Can you please suggest the medicine and treatment for a stage four pressure ulcer on the hip? It is a few inches above the buttocks.

Thank you.

Hi,

Welcome to icliniq.com.

Thank you for writing, and I understand your concern. This is a bedsore seen in bedridden patients, so proper precautions and care are required. And you can take the surgery reference once. I would suggest you do the following things,

1) The patient should sleep on an air mattress.

2) Change positions every two to three hours.

3) Proper cleaning is required with sterile gloves using Betadine and Hydrogen peroxide and dressing on alternate days.

4) Tablet Augmentin 625 duo (Amoxicillin 500 mg and Clavulanic acid 125 mg) three times a day after a meal. (if he is not taking any antibiotics)

Is he taking any antibiotics currently? Also, since when has the bedsore been present?

Please let me know.

I hope that this answers your query.

Kindly follow up if you have more doubts.

Thank you.

Thank you.

Patient's Query

Hi doctor,

Thank you for responding.

No, he is not taking any oral medications as of the moment. Is there any specific one we can use for the dressing, or is a normal one fine? He has this bedsore for about two weeks.

Thank you.

Hi,

Welcome back to icliniq.com.

Thanks for the quick response. It will take time to heal because the bedsore is large. For the dressing, you have to use sterile surgical gloves and buy a sterile gauze piece. It should be done in a sterile way. You can watch videos on the internet, or you can take him to a nearby clinic and get it done.

I hope that this answers your query.

Kindly follow up if you have more doubts.

Thank you.
