Hello doctor,

I am confused about metabolic surgery options. My endocrinologist is recommending gastric bypass for my type 2 diabetes (A1C 9.4), but the surgeon is suggesting sleeve gastrectomy instead. My BMI is only 34, so the primary goal is to improve my diabetes. I am currently on maximum-dose metformin, GLP-1 injections, and will be starting insulin soon.

In simple terms, why might gastric bypass be better for diabetes, even though it carries more risk for complications? The surgeon emphasizes that the sleeve has a lower complication rate, while the endocrinologist insists that the bypass provides a stronger "metabolic effect," regardless of weight loss differences.

I need a clear explanation of how each surgery specifically affects diabetes physiology beyond just weight loss. It is difficult to compare the conflicting recommendations when each specialist favors a different approach. Gastric bypass surgery alters the gastrointestinal tract by causing food to bypass most of the stomach and part of the small intestine. According to the American Society for Metabolic and Bariatric Surgery (ASMBS), this procedure, also called Roux-en-Y gastric bypass, leads to remission of type 2 diabetes in nearly 80 percent of patients, due to both weight loss and additional metabolic changes.

Whereas, sleeve gastrectomy involves removing a large portion of the stomach, leaving behind a smaller, tube-shaped stomach. This surgery also causes metabolic and hormonal changes that can improve blood sugar control. According to the ASMBS, more than 60 percent of patients who undergo sleeve gastrectomy achieve remission of type 2 diabetes.

The choice between procedures often depends on the extent to which obesity affects your health. If you are dealing with multiple obesity-related conditions, such as metabolic syndrome, or if excess weight significantly impacts your mobility, gastric bypass may offer better results. In clinical comparisons, remission of type 2 diabetes was achieved in 93 percent of patients who underwent gastric bypass, compared to 47 percent in the sleeve gastrectomy group. Patients who had a gastric bypass also experienced greater weight loss, reduced waist circumference, and lower blood glucose, HbA1c (glycated haemoglobin), and lipid levels compared to those who had a sleeve gastrectomy.

The sleeve gastrectomy is a simpler procedure than the gastric bypass. It typically results in shorter operating times and may carry a lower risk of complications. Given your situation, gastric bypass appears to be more effective for managing your diabetes, although it is a more complex procedure.
