Hi doctor,

It has been more than two years since it happened. My penis's inside covering has a fungus infection again. I used Candid-B for ten days, and even then, there has been no change. Please suggest a solution.

I have seen your query and concern.

With your descriptions, it seems you have balanoposthitis. Do not use Candid-B (Beclamethossone and Clotrimazole) as it contains steroids. Balanitis is inflammation of the glans penis. Balanitis involving the foreskin and prepuce is termed balanoposthitis. Predisposing factors include poor hygiene and overwashing, use of over-the-counter medications, and non-retraction of the foreskin. It may be associated with diabetes and candidal fungal infections. I suggest the investigations like blood sugar and KOH (potassium hydroxide) mount for candidiasis. Suggested treatment measures include:

  1. Gentle retraction of the foreskin daily and soak in warm water to clean the penis and foreskin.
  2. You can use a combination of Fusidic acid and Clotrimazole twice daily for one month. If balanitis does not respond to topical therapy, then you can take Ketoconazole 200 mg twice daily for two weeks.
  3. In recurrent cases, 1 % Pimecrolimus cream should be used instead of steroids.
