Hello doctor,
I have a rash under the foreskin of my penis. The rash is on the shaft under the foreskin and not on the tip of the penis. It is not itchy, but it feels very dry and scaly. I applied steroid cream to the area to see if it would help, but it did not have an effect. I have had the same sexual partner for the last six months, and I do not think it is an STD. I have recently moved to a new city with hard water and have been suffering from eczema and acne flare-ups from the hard water here. I had fungal infections such as jock itch, but that was years ago. Can you please suggest the reason for this and what treatment I should use to treat it? Thank you.

I read your query and understand your concern. As per the history and image provided (attachment removed to protect the patient's identity), it is candida balanitis. It is a form of inflammation under the foreskin mainly triggered due to fungal infection. The most common symptoms which may appear are dry white patches with occasional itching and pain, redness, and discomfort. However, some individuals may not experience typical symptoms but may experience discomfort, dryness, scaly lesions, and redness. My suggestions are (Consult a specialist doctor, talk with them and take medications with their consent) apply Miconazole gel (topical anti-fungal medication) on the affected area twice daily for six weeks or till the rashes fade after cleaning and towel drying the area. Avoid scrubbing and using soap, shampoos, or harsh chemicals to clean the area. Use lukewarm water to wash and clean the area and always towel dry the area before applying the medication. Apply light liquid paraffin emollient on the area after bathing. It acts as a moisturizer to retain hydration and prevents dryness and scaling of the affected area. Kindly follow the above advice and do a follow-up in a month. I hope this has helped you. Thank you.
