Can balanitis cause marks on the inner foreskin?

Hi doctor,

I had redness on the inner foreskin diagnosed as balanitis. There is no STI (sexual transmitted infection) or anything like that. My doctor prescribed Locoid steroid cream to treat it but it has stayed persistent. Also, some marks have appeared on my inner foreskin. What could this be? Should I be worried?

Thanks for the reply.

So this does not look like HPV in any way? There is no itching or burning, but there is a slight discomfort feeling. I have only used steroid cream for two weeks now. I have not tested for syphilis, but does it seem likely it could be that? It does not hurt too much.

Thanks for the reply.

I had unprotected sex, so that might be considered high risk. I have got some type of radiating pain going on at the same time through my groin and also down into the testicles. The redness and groin pain started about three months ago. A couple of months after the unprotected sex. Now it has developed into more distinct rashes as the picture shows. Please help.

There are two differential diagnoses. It could be genital herpes or Syphilis.

  1. Take a tablet of Zifi (Cefixime)200 mg twice daily after food for five days.
  2. Wash the area with a non-soap cleanser daily for two months.
  3. Apply Fucidin cream twice daily in the morning and night for seven days.
  4. Keep the area clean and dry.
  5. Do not scratch or rub the lesions.
  6. Maintain proper genital hygiene.
  7. Always wash your genitals after sexual intercourse.
  8. Practice safe sex.
  9. Use barriers.
  10. Get urine routine and culture microscopy done.
  11. Get HSV (herpes simplex virus) 2 and VDRL (venereal disease research laboratory) test done to rule out possible sexually transmitted infection.

I will go to a dermatologist to take tests from the area. If it is herpes, would not the lesions or areas that you saw burn or sting?
