Hi doctor,

Four days back, I had back pain radiating to the abdomen on the left side. I have been experiencing similar pain for ten days below my right thigh. So I was under the impression that it could be muscle pain, so I took a hot water bottle. But I did not get relief. Finally, I went to an orthopedic, but he suggested going for a kidney, ureter, and bladder X-ray. The doctor found a few small stones in my left kidney. The size of the prostate also increased, and he suspected some prostate inflammation and suggested to take some antibiotics like Livofloxacin 750 mg once a day for seven days. I have seen in my blood reports that my sugar has gone up. Fasting was 103 mg/dL, and post-prandial was 177 mg/dL. When I got it tested today, fasting was 95 mg/dL, and post-prandial was 171 mg/dL. Today I met the physician for my body pain, and I could observe a few infections on the skin, a cyst on the arm of the left hand, and another cyst below the lower abdomen on the left side and near the penis. It seems to be some lymphocyte infection. My physician suggested that it looked like herpes and asked to take immediately following treatment.

The doctor suggested me to apply Acyclovir cream, and use tablet Acyclovir 400 mg thrice daily and Trptomer 25 mg for seven days. At present, I am taking the following medicines for blood pressure. I have had blood pressure for the last 40 years, which is hereditary. I take Cilaca 10 mg morning and at night. Nowadays, my blood pressure remains normal. What shall I do for sugar? For fatty liver any precaution required or not? Urologist has prescribed me Urimax 0.4 mg as I had passed urine at night several times which disturbed my sleep. I have my regular check-up, but nothing significant is found. My physician wants me to start the Urimax course after completing the herpes course of seven days. Please provide me with your valuable suggestion.

I understand your concern.

It seems like the pain you were experiencing is either due to the shingles or it can be musculoskeletal pain. It is improbable to be due to kidney stones or prostate enlargement. You have impaired glucose tolerance. Start with a strict sugar-free, low-carbohydrate diet and regular physical activity. Give it a try for three months and repeat blood tests, and if it is still high, I suggest you start Metformin. For fatty liver, diet and exercise is the primary treatment stay. Weight loss is essential, and do not take alcohol. As you pass urine often at night, you will benefit from taking Urimax.

Thanks for the reply.

I had my blood test, and the following abnormal readings are as follows. Fasting blood sugar was 106 mg/dL, HbA1C was 6.6 %, serum creatinine was 1.22 mg/dL, and ESR was 27 mm/hour. After some days, I had my blood sugar readings in digital glucometers, and fasting blood sugar was 94 mg/dL, and post-prandial sugar was 152 mg/dL. I had my blood parameters from another laboratory, and their readings were fasting blood sugar was 115 mg/dL, and HbA1C was 6.4 %. Mean plasma glucose was 136.98 mg/dL, and serum creatinine was 0.87 mg/dL. Today, I had my blood sugar tested in a digital glucometer and found fasting blood sugar was 82 mg/dL, and post-prandial sugar was 118 mg/dL.

My concern is about sugar. Of course, I was very careless and also took sweets. There was no control due to festivals. At present, I am not taking any medicines for sugar. So I should be allowed to improve my lifestyle instead of adding medications. This time my thyroid, TSH (thyroid stimulating hormone), is normal. I am taking Thyroxine 0.25 mg once daily. I am taking the following blood pressure medicines: Telmisartan 40 mg, and Cilnidipine 10 mg, one tablet in the morning and at night. I have completed one month's course of Urimax, but the problem of passing urine several times at night is still not solved.

I also had my ultrasound, in which the following impression was found that my gall bladder is partially contracted. Hepatomegaly with grade-II fatty liver and bilateral renal concretions were there. The prostate size is 0.7 oz, after which I have taken Urimax medicines. My taste is terrible in the morning when I get up—any precaution, as serum creatinine shows borderline readings. Please advise step by step to understand better and follow the regime.

I understand your concern.

You do not need any medicines for sugar. Continue with diet and walking. If TSH (thyroid stimulating hormone) is normal, continue the same dose of Thyroxine. If Urimax (Tamsulosin) is not relieving the prostate symptoms, even though PSA (prostate-specific antigen) is normal and ultrasound does not show any concerning findings, still better to see a urologist. Until then, continue Urimax daily at night. The fatty liver will improve with some weight loss. If you need sugar medicines, Pioglitazone will be an excellent choice to target sugar and fatty liver. There is no concern about creatinine. Kindly check the urine microalbumin creatinine ratio next time. Continue the same blood pressure medicines.
