Should additional tests be considered if the ANA test is negative?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello Doctor,

I am being screened for autoimmune diseases. My sedimentation rate was 65 for about two months. All symptoms, according to my rheumatologist, point to lupus, but I have not been diagnosed yet. He had to run blood work again. I have severe anemia, hair loss, mouth sores that come and go, and muscle and joint pain. I received my blood work results yesterday, and my alpha-2 globulin is high. However, my sedimentation rate dropped to 11, and my ANA (Antinuclear Antibody) screen was negative. I am in pain daily, and my body hurts significantly. I am also experiencing some memory issues.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

I have reviewed your symptoms. The fatigue, joint, and body aches, and oral ulcers suggest an autoimmune etiology. However, as your ANA(Antinuclear Antibody) test has come back negative, the condition has not overtly manifested at this time. The increase in alpha-2 globulin also indicates immune activation.

In light of these findings, I would like to ask if you have experienced dryness of the eyes, dryness of the mouth, skin rashes, photosensitivity, or frequent fevers.

If possible, please consider getting a C-reactive protein (CRP) test and an ANA profile test, also known as the ENA(Extractable Nuclear Antigen test) test. However, at present, I do not believe there is a need to start any medication for lupus. I recommend continuing the medications prescribed for fibromyalgia. You may also take Tylenol (Analgesic and antipyretic) if the pain becomes too severe.

Additionally, I encourage you to consult an autoimmune specialist to address any doubts or concerns you may have regarding your condition.

I hope this helps.

Thank you.
