Hello doctor,
I have had severe anxiety, fast heartbeat, palpitation, depression, and IBS diarrhea for the last 20 years. I tried various medicines, Ayurvedic, Allopathic, and Homeopathic but no results. Most psychiatric medicines do not suit me. Presently I have consulted one psychiatrist and he suggested some medicine: Nexito Forte, Pronol H, and Nortimer 25 mg all twice a day.
I have been taking the above medicine for the last six months. The above medicine suits me well and controls my IBS diarrhea problem. But my anxiety issue is not controlling. I still feel anxiety, palpitation, and sweating issues and avoid social gatherings. If I increase Mortimer it causes an increase in palpitation and sweating issues, and if I increase Propranolol it increases stomach upset and diarrhea issues. My doctor suggested I add Desvenlafaxine 50 mg. Should I add Desvenlafaxine along with Nortriptyline or I just go with Nexito forte and Nortriptyline tablet? Long-term effect of Nortriptyline on my body? In the past, I used Desvenlafaxine 50 mg but it caused too much sweating and palpitation issues with a bit of stomach upset. I also tried Paroxetine, Veniz XR, Duloxetine, Prozac, and Mirtaz but not much medicine suits me. Please suggest as I am confused with too many medicines.

Nortriptyline is a good medication and it has side effects like dry mouth, blurring vision, and constipation. You can take it for a long time without any worries. You need to have faith in your doctor. You can take any of the above medications which will help you. Desvenlafaxine is also good for anxiety and depression. But I prefer tablets Nexito forte (Clonazepam and Escitalopram Oxalate) with Nortriptyline as it is my choice of drug for a person who has a problem like yours. I suggest you try some medication like the tablet Isozex (Isopropamide and Trifluoperazine) for IBS (irritable bowel syndrome) and the tablet Zepox CD (Clidinium and Dicyclomine). Do meditation and learn stress management. You can go for a biofeedback to your nearest psychiatrist or neuro physiotherapist; it will help you to relax and manage IBS. Do not just concentrate on medicine treatment; there are other alternatives for IBS and anxiety. Please let me know if you need any help.

I am already taking Isozex in the morning. My confusion is, if I add Desvenlafaxine 50 mg along with Nexito forte and Nortimer 25 mg, will that help to control my anxiety and depression? Or will it increase my IBS-diarrhea, palpitation, and sweating problem? What do you suggest? Should I add Desvenlafaxine 50 mg or not for my problem? Please suggest.

You can take Desvenlafaxine with Nexito forte and Nortriptyline. It will not worsen your IBS. You should take a tablet of Isozex at night, it will help better. You should do meditation and manage your diet with the help of a dietician. I hope that you will find this helpful.
