Patient's Query
Hi doctor,
I need urgent advice about my hereditary angioedema. I am 38 and was diagnosed three years ago after multiple unexplained swelling episodes, and found out my dad had it too, but never got diagnosed. My C1-inhibitor levels are very low (14%, normal is above 70%), and I am getting attacks more frequently now, three in the last month. My last attack affected my throat and I almost could not breathe. I had to go to the ER, where they gave me an Icatibant injection. I am terrified of having another throat attack when I am alone. My allergist prescribed C1 esterase inhibitor concentrate to keep at home, but it is extremely expensive, and my insurance only covers four doses per month.
I also have a lot of abdominal attacks with severe pain that have been mistaken for appendicitis twice. Hereditary angioedema seems to be triggered by stress and minor injuries. Could I benefit from preventive treatment instead of just treating attacks? I read about Lanadelumab, but I do not know much about it. My daughter is 12 and showing signs of having hereditary angioedema, too. She had hand swelling last week that lasted two days. What should we do to manage this condition better?
Please help.
Hi,
Welcome to icliniq.com
I have read your query and can understand your concern.
Hereditary angioedema (HAE) is unpredictable, but there are solid steps you can take to reduce risk and gain better control.
Your current situation:
HAE type I (low C1-inhibitor level and function).
A life-threatening throat attack recently needed emergency Icatibant.
Frequent attacks (three this month), including severe abdominal ones.
A limited supply of on-demand therapy due to insurance (only 4 C1-INH doses per month).
Your daughter (age 12) may also be showing early symptoms.
Are you a candidate for preventive treatment?
Yes, you could benefit from long-term preventive (prophylactic) treatment rather than relying only on emergency meds. It helps reduce attack frequency by over eighty percent. It is especially helpful for people with frequent or severe throat or abdominal attacks. Safe and approved for patients more than 12 years old. Also effective, especially if you already respond well to on-demand C1-INH. You and your allergist or immunologist can choose based on your attack frequency, lifestyle, and insurance coverage.
I hope this answers your questions.
Thank you and regards.
