Hello,

I do not have ejaculation anymore. I have no obesity or loss of facial hair. I am suffering from depression and low mood, along with a foggy brain. Please help.

I read your query and understand your concern.

Your symptoms suggest anejaculation. Anejaculation can be due to retrograde ejaculation, where semen does not come out, or it will go back where it comes from. I suggest the following tests to rule out the causes:

1. Increased blood sugar - you must check for blood sugar.

2. Prostate issue – you must plan for an ultrasound pelvis.

3. Renal stone – You must plan for an ultrasound of the abdomen.

Also, answer these for better clarity:

1. Is there any history of surgery, or do you take any medication related to prostate problems?

2. Have you had any spinal cord injury?

3. Do you have anxiety?

Treatment can be made easy if we know the cause of the issue. In general, medication would not be of much use in this situation. Other options include penile vibratory stimulation and treatment for retrograde ejaculation.

Thank you for your reply.

Seven months back, I had high levels of ejaculation and no erection issues. I am a muscular person; my erection is hard, but I feel depressed. I do not have gynecomastia or female features. I have always been sexually active, but recently, I have had a total loss of libido. I never had such issues, and my sperm volume was very good. I do not have facial beard loss or anything like that. This happened very suddenly. I have been through heavy stress and some depression. But testosterone being almost zero is something no one has ever seen. I have checked three different urologists.

I understand your concern.

In your reports (attachment removed to protect patient identity), I can see low testosterone and varicocele in scrotum calcification. For testosterone production, your testicles need to work properly. The report says there is varicocele. We need to correct it and find the cause of calcification. Secondly, you need the hormones FSH (follicular stimulating hormone) and LH (luteinizing hormone) for the production of testosterone. So, you need to check your FSH and LH hormone levels. You also need to do a skull X-ray or CT (computed tomography) of the brain, as these hormones are produced in the brain. I suggest taking synthetic testosterone once a day before food for 30 days as a temporary solution.
