Hello doctor,
I am male, 51 years old. I have diabetes (type 2), high blood pressure, medicines are controlling it all. Have two herniated discs, neck and back. The problem is anal issues for the last few weeks. Extreme pain. Worst in life. It feels like a hot knife slicing me down there. Sometimes, rarely, some blood is there. When I apply the cream, a small, bulb-like protrusion is there. Currently, I am taking Metformin, Lisinopril Depakote.

It appears you have a fissure. The first thing is your stool should be soft. For that, you should eat high fiber diet like fruits, apple, banana, oranges, green leafy vegetables like spinach, raw cucumber. Avoid smoking, refined food, fatty, and oily food. You should have plenty of fluids and regular exercises. Also, syrup like Lactulose 15 to 30 ml bedtime to maintain your stool soft.
Secondly, you can apply 2 % Lidocaine gel before each stool to relieve pain. Also,topical Nitroglycerin typically applied rectally as a 0.2 or 0.4 percent ointment twice daily for eight weeks will help in healing. Sitz bath will be helpful, during a sitz bath, the anus is immersed in warm water for approximately 10 to 15 minutes two to three times daily. Sitz bath kits are available in most drugstores which contain Potassium permanganate. If it does not help, you need to visit a surgeon for examination and management.
