How can one manage breast cancer and Addison’s disease?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been diagnosed with stage 2 breast cancer with Addison’s disease. I have had two adrenal crises since starting chemotherapy. The stress-dose steroids make my diabetes worse, but I cannot skip them.

Also, I have a salt craving and constant dizziness. During treatment, blood pressure drops so low that they almost had to stop it last time. So my questions are:

  • How do we manage this dangerous combination?
  • What integrative approach has to be taken so that all the conditions can be better regulated?

Kindly help.

Hello,

Welcome to icliniq.com.

I can understand your concern.

Based on your description and condition, immediate concerns include managing adrenal crises, managing blood pressure fluctuations, and symptoms like salt cravings and dizziness. It can be done as follows:

  1. Continuing Hydrocortisone (Cortef) or Prednisone (Deltasone) for stress-dose steroids is essential, but dosage adjustments may help minimize the impact on diabetes.
  2. Blood pressure should be closely monitored during treatment, with possible adjustments to Fludrocortisone (Florinef) or IV fluids to prevent severe hypotension.
  3. Salt cravings and dizziness may indicate the need for increased sodium intake, but this should be balanced carefully with blood pressure monitoring.

An integrative approach should involve a multidisciplinary care team that includes oncologists, endocrinologists, and primary care physicians. This team can coordinate a comprehensive treatment plan to balance cancer treatment, adrenal insufficiency, and diabetes management. Personalized medication adjustments for steroids, Insulin, and antihypertensive medications should be reviewed regularly to minimize interactions and optimize dosages.

Lifestyle modifications can support overall health and symptom management. This includes:

  1. A balanced diet with whole, nutrient-dense foods is crucial for stabilizing blood sugar and preventing excessive inflammation.
  2. Stress management techniques, such as meditation or yoga, may help mitigate adrenal insufficiency symptoms and support overall well-being.
  3. Regular, low-intensity exercise, like walking or swimming, can help maintain stability in blood sugar and blood pressure while improving overall endurance and strength.
  4. Close monitoring is essential to detect changes early.
  5. Regular tracking of vital signs, blood glucose levels, and adrenal function can help prevent complications.
  6. Frequent communication with healthcare providers ensures timely adjustments to medications and treatments, improving the overall regulation of all conditions.

I hope this helps.

Thank you.
