Hello doctor,

I had suffered from severe waist, pelvic, and lap pain during and after menstruation. The pelvic scan done previously showed endometriosis. Another scan that I had got done recently showed adenomyosis. However, the pain still persists. Please suggest the treatment.

Adenomyosis and endometriosis both need some hormonal support. Endometriosis can be treated maximally via laparoscopy, whereas adenomyosis does not have any proper treatment. Even if the patient is nearing menopause, it is advised to remove the uterus along with tubes and ovaries if the symptoms are severe. Fibroid if large, obstructing the uterine cavity or cervix or affecting symptomatically, needs myomectomy (removal of fibroids). I suggest you the instructions mentioned below,

1. Take cyclic combined oral contraceptive pills for three months for hormonal support to alleviate symptoms, but it is advised not to be taken for very long as you have adenomyosis, fibroids, and endometriosis.

2. Take a tablet of Mefenamic acid (a non-steroidal anti-inflammatory drug) 500 milligrams every eight hours to minimize pain for three to five days starting from day two of your period cycle.

Kindly consult a specialist doctor, talk with them, and take medicines with their consent.
