Introduction

MiniMed 780 G is the latest version of the insulin pump introduced by Medtronic for people of seven years and above to manage type 1 diabetes. The device helps individuals monitor their glucose levels continously through trends and patterns. It then delivers the insulin automatically through this insulin pump and adjusts the dosage according to need.

MiniMed 78G system by Medtronic was approved by the United States FDA (Food and Drug Administration) in April 2023 for people over seven years suffering from type 1 diabetes. The approval for this device was submitted in the year 2021, but it got delayed due to some quality control issues at the manufacturing unit of Medtronic. However, MiniMed 780 G was approved by Canada in November 2022 and Europe in 2020.

What Are the Features of the MiniMed 780 G System?

The features of MiniMed 780 G system has the following features:

  • SmartGuard: This device operates on SmartGuard technology that can automatically adjust the insulin pump’s delivery up to 288 times a day and every five minutes by continuously monitoring the glucose readings. This advanced feature is a major improvement in the MiniMed 670 G system. In addition, this Smart Guard autocorrection technology also helps to compensate for inaccurate carbohydrate readings.

  • MiniMed Mobile Application: The device can also be connected to Android or Apple smartphones to view glucose levels. These mobile applications allow people to track their blood glucose levels and provide an alert for highs or lows.

  • Automatic Correction Boluses Feature: The latest version of the insulin pump, MiniMed 780 G, has automatic correction boluses, which were lacking in the previous 770 G pump.

  • Adjustable Diabetes Goals: The default blood glucose range is targeted to be 120 mg/dL (milligram per deciliter), but it can also be adjusted to 110 or 100 mg/dL in this device.

  • CareLink Connect Application: The real-time diabetes data can be shared with five CareLink partners through the CareLink application. This provides them access to track a person’s glucose levels and information on their insulin pump remotely to ensure that the person is fine.

How Does the MiniMed 780 G System Work?

  • The MiniMed 780 G consists of a sensory wire inserted under the skin on the person’s abdomen to measure blood glucose levels in the body's tissue fluids. The obtained values are sent wirelessly to BlueTooth Low Energy (BLE) and then to the insulin pump and displayed on the screen along with glucose alerts, trend information, and alarms. The insulin pump infusion set then delivers the insulin in a calculated dose.

  • The device also alerts the user when they are hyperglycemic (blood glucose levels are dangerously high) or hypoglycemic (blood sugar levels are dangerously low). This helps diabetes people gather their glucose information through continuous glucose monitoring and know the pattern of their glucose levels. They also use the delivered insulin from this pump to keep their glucose within a safe range.

  • The MiniMed 780 G system also assists in diabetes management by automatically delivering insulin from the pump based on changing glucose levels.

What Are the Devices Connected to the MiniMed 780 G System?

The MiniMed 780 G is composed of three devices that are connected.

These include:

  • Continuous Glucose Monitoring (CGM) - The Guardian 4 Sensor CGM helps to measure the glucose levels every five minutes and then sends the reading to the insulin pump. The CGM needs to be replaced after every seven days.

  • Infusion Set - An advanced infusion set (MiniMed Mio) consists of a pre-loaded insertion device, an automatic needed retraction, and a 90-degree soft cannula. The infusion set can be handled single-handedly and needs to be changed every two to three days.

  • Insulin Pump - It is a waterproof 780 G pump that can automatically adjust and correct this insulin delivery system. It releases more insulin when the glucose level is high and less when the levels are low.

What Are the Accomplishments of the MiniMed 780 G System?

The MiniMed 780 G insulin pump has shown its safety and effectiveness in real-time and clinical study data for use in children and adults seven years and older suffering from type 1 diabetes. In clinical trials, 329 participants between the ages of seven and 75 who had type 1 diabetes were included. These participants used Guardian Sensor 3 with an insulin pump and Advanced Hybrid Closed Loop algorithm.

The data for real-time was obtained from a continued access study on 176 participants, using both the MiniMed 780 G system for the first time with a hybrid closed loop system and having previous experience with the Advanced Hybrid Closed Loop algorithm, with prior use of the Minimed 780 G with Guardian Sensor 3 or 4.

When Should the MiniMed 780 G System Not Be Used?

The MiniMed 780 G system should not be used in the following situations:

  • If someone cannot hear, see, or recognize the pump alerts, signals, and alarms.

  • Those who cannot stay in contact with their healthcare provider willingly or unwillingly.

  • The insulin pump should not be kept in magnetic cases or exposed to any magnet.

  • It should also be avoided from X-rays, MRI (magnetic resonance imaging), computed tomography (CT), or diathermy equipment. One must remember to remove the sensory, pump, meter, and transmitter before entering the area with all this equipment.

  • Infusion sets are not recommended for intravenous (IV) infusions. Therefore, the device should only be used for subcutaneous infusions.

  • Also, the infusion sets and reservoir should not be used for blood and blood product infusion but only for insulin infusion.

Conclusion

The MiniMed 780 G insulin pump is a battery-operated, ambulatory, micro-infusion pump for insulin delivery. The device can be operated in both auto and manual (auto-insulin delivery mode is suspended), and they have their respective capabilities and features. The device is intended to monitor the glucose levels continuously in the fluid under the skin, then deliver the insulin based on these observed values and predicted falls and highs. The MiniMed 780 G system is the first ever invented device that can address the needs of real-time automatic insulin correction in people with type 1 diabetes.
